A LEVEL ONE FINGERPRINT CLEARANCE CARD IS REQUIRED. THE COST TO ROLL PRINTS IS USUALLY $15-20 AND THE APPLICATION IS $67. PLEASE THESE INSTRUCTIONS TO REQUEST YOUR FINGERPRINT CLEARANCE CARD:
1. Click “Request a Replacement / Apply for a Card” and “Continue”
2. Create a new Login by clicking “It’s easy to create one.” (If you have one, just login and go to step 4.)
a. Select “Individual Personal Account” and continue
b. Fill in your information and continue for the next few screens and click “create account”
c. Verify your email by clicking on the link in the email that was sent to you
d. Now you can login to the site
3. Once you are logged in select “Fingerprint Clearance Card” and continue.
4. Apply for a Non-IVP Card
a. Select “DES - DDD/HCBS - Home and Community Based Services ARS § 36-594.01” for the reason for applying.
b. Select “Employee” for the question if the person is an employee or volunteer.
c. Follow the steps to input your information, sign, and review.
d. Make the payment for $67 and take note of your reference number and bring it to get your prints rolled.
5. Make an appointment to have your prints rolled at a professional fingerprinting center. We recommend Arizona Livescan.

The following courses are provided to caregivers. Each year, per compliance with the State of Arizona, caregivers will complete an additional 6 hours of training. Each of these courses represents an hour.
