FINGERPRINT CLEARANCE CARD
A level one Fingerprint Clearance Card is required. The cost to roll prints is usually $15-20 and the application is $67. Please follow these instructions to request your Fingerprint Clearance Card.
- Click "Request a Replacement / Apply for a Card" and "Continue".
- Create a new login by clicking "It's easy to create one," (if you have one already, login in and skip to step 8).
- Select "Individual Personal Account" and continue.
- Fill in your information and continue for the next few screens and finally click "Create account".
- Verify your email by clicking on the link the email that was sent to you.
- Now you can login to the site.
- Once you are logged in, select "Fingerprint Clearance Card" and continue.
- Apply for a Non-IVP Card
- Select "DES - DDD/HCBS - Home and Community Based Services ARS § 36-594.01" for the reason for applying.
- Select "Employee" for the question if the person is an employee or volunteer.
- Follow the steps to input your information, sign, and review.
- Make the payments for $67 and take note of your reference number and bring it to get your prints rolled.
- Make an appointment to have your prints rolled at a professional fingerprinting center. We recommend Arizona Livescan.
TRAINING & CERTIFICATIONS
Direct care workers are required to be trained in first aid, CPR, and Article 9. Here's a list of local training companies that offer these classes:
